We made a decision right from the start to work with Support Coordinators (and others) in a wrap-around model.

So, here’s what to expect from us – This short piece lays out how we feel about the environment we work in, and how we have a part to play, alongside other providers, in helping our clients to get the best outcomes.

When it comes to providing comprehensive disability services, it is critical to collaborate closely with others. Service providers can ensure that individuals with disabilities receive the support and assistance they require to live fulfilling and independent lives by working together.

The use of a team approach is the only effective way to work collaboratively. This entails bringing together a diverse group of professionals, such as social workers, occupational therapists, and case managers, to collaborate in order to meet each individual’s unique needs. Under the NDIS, this often occurs under the supervision of dedicated Support Coordinators.

Previously, the location of the majority, if not all, services within State-run entities allowed for the use of a coordinated care model. This entailed developing a comprehensive plan that addressed all aspects of a person’s life, including medical, social, and educational needs. Individuals would then receive the support they require to thrive and reach their full potential if care was coordinated across multiple service providers.

Now that the situation has changed, we are striving to combine the best features of the new “choice and control” NDIS environment with the best features of the previous regime. While it was sometimes lost in red tape, effective communication is also essential for successful collaboration. We believe service providers should work together to ensure that everyone is on the same page and that there is a clear understanding of each individual’s needs and goals. It doesn’t matter how – regular meetings, communication protocols, or shared documentation for example – but it must happen.

Of course, we must involve the participants in the collaborative process. By actively engaging individuals and their families in the decision-making process, we can ensure that the services being provided are tailored to meet their unique needs and preferences, without duplication, and in a cost-effective manner.

We are totally committed to being flexible and responsive, and to working with other service providers, without ego, preconceptions, or unnecessary turf warfare to ensure that individuals with disabilities receive the support and assistance they need to lead fulfilling and independent lives.
