Mentoring & Community Support

Mentoring and community support for NDIS participants in Southern Adelaide

A consistent, caring support worker who shows up – to help you build confidence, connection, and independence in your everyday life.

What We Do

Real support for real life

Mentoring and community support is one of the most flexible NDIS supports available. It’s about having a consistent, trusted person alongside you to help you learn new skills, get out into the community, and build a life that matters to you.

At Heartfelt Support, our support workers aren’t just task-completers. They’re people who show up with genuine care, remember the details that matter, and build real relationships with participants over time.

  • One-on-one mentoring to build life skills and confidence
  • Community access – getting to events, services, and social activities
  • Social skills development and peer connection
  • Help navigating daily routines and building structure
  • Transport and accompaniment to appointments
  • Support to pursue hobbies, interests, and goals
  • Linkage to other community groups and activities
Who It’s For

Who benefits from mentoring support

Social isolation

If you’re spending too much time alone, a support worker can help you get out, build connections, and rediscover the parts of community life that matter to you.

Building independence

Whether it’s catching the bus, managing money, cooking meals, or navigating public spaces – mentoring support helps you build skills at your own pace.

Psychosocial disability

Recovery often happens in the community, not in a clinic. A consistent mentor alongside you helps build routine, confidence, and social connection.

Young adults in transition

The period from school to adult life can be challenging. Mentoring support helps young people build independence, social skills, and find their place.
